Service Forms for Prospective Students
Statement of Financial Support - Use this form to document your finances. This form can be used by the student and by other financial sponsors. Remember, current students, if accepted to a masters degree or other new program, must also submit new financial documents.
Transfer Form - For students transferring from another school to Eastern University.
Health Forms - Required for all incoming international students in all programs. More information is available on the Student Health Center webpage.
