Van Request Form

Eastern University athletic teams, clubs, campus organizations, faculty / staff members, and students may use this form to request usage of University Vans.

SUBMISSION OF THIS FORM MERELY CONSTITUTES A REQUEST AND DOES NOT GUARANTEE THAT UNIVERSITY VEHICLES WILL BE AVAILABLE TO YOU DURING THE TIME FRAME THAT YOU SPECIFY.

    * Submit your request via this form as early as possible in advance of your event.
    * If all Vans are booked when you make your request, your organization will have to rent a van from one of Eastern's Preferred Car Rental Vendors.
    * Each group should have a contingency plan if the van they reserved is canceled due to maintenance issues, weather problems, or other unexpected 'Acts of God'.  A CONFIRMED RESERVATION IS NEVER A GUARANTEE.
    * Please fill in ALL requested information. Your van will not be booked until ALL requested information is submitted.
