Step 1
Fill out the Request for Accommodations form.
- For Academic, Mobility/Parking, Housing, and Meal Plan accommodations, students must use their Eastern issued email account to access the link.
- Students will be directed to a Google Form asking for the following information:
- Student Information (ex: student ID, address, phone number, etc.)
- Student diagnosis and date of diagnosis
- List of requested accommodations
Step 2
Provide Healthcare Provider Documentation
- In order to complete the request process, students must attach their Healthcare provider documentation or have their healthcare provider fax/email their documentation to ODA. The Healthcare Provider Documentation forms can be found on ODA’s Policies and Procedures page (eastern.edu/oda) and can only be viewed by Eastern email addresses. If you are a guardian or someone handling paperwork in collaboration with a student, please email ODA (oda@eastern.edu).
Step 3
Schedule an Accommodation Intake meeting
- After filling out the above form and sending in the healthcare provider information, the student can email ODA to schedule a meeting (oda@eastern.edu). If you would like a paper/PDF copy of the forms, please email ODA (oda@eastern.edu). If you need assistance with filling out the forms, please email ODA (oda@eastern.edu) .
Support & Service Animals
For an Emotional Support Animal accommodation and Service Animal notification, please schedule a meeting with Kelsey Fiander-Carr from ODA. Access Kelsey's Booking calendar. This meeting will discuss the policy and procedure for animals required due to a disability. Paperwork will be distributed to the student after the initial meeting.
